Job Summary
Grace Senior Care is seeking a compassionate, dependable, and experienced Registered Nurse (RN) to join our Adult Medical Day Care team on a per diem basis. The Registered Nurse is responsible for providing high-quality nursing care, monitoring participants' health status, coordinating with physicians and caregivers, maintaining accurate clinical documentation, and ensuring compliance with New Jersey Department of Health regulations.

Responsibilities
- Perform nursing assessments and ongoing participant evaluations.
- Monitor participants' medical conditions and report significant changes to physicians and family members as appropriate.
- Administer medications and treatments in accordance with physician orders and facility policies.
- Maintain accurate clinical documentation and participant records.
- Develop and update individualized care plans.
- Respond to medical emergencies and provide appropriate nursing interventions.
- Monitor infection prevention and control practices.
- Educate participants and caregivers regarding health conditions and wellness.
- Collaborate with physicians, social workers, therapists, activity staff, and other interdisciplinary team members.
- Ensure compliance with New Jersey Department of Health regulations, HIPAA, OSHA, and facility policies.
- Participate in quality improvement activities and staff meetings as needed.
- Perform other duties assigned by the Director of Nursing or Administrator.
Qualifications
Required
- Current New Jersey Registered Nurse (RN) License in good standing.
- Current CPR/BLS Certification.
- Strong clinical assessment and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Ability to maintain participant confidentiality.
Preferred
- Experience in Adult Medical Day Care, Adult Day Health Services, geriatrics, rehabilitation, long-term care, home health, or medical-surgical nursing.
- Experience with electronic medical records (EMR).
Physical Requirements
- Ability to stand and walk for extended periods.
- Ability to assist participants with mobility when necessary.
- Ability to lift up to 25 pounds with or without reasonable accommodation.
Schedule
- Per Diem
- Day Shift
- Monday–Friday
- No weekends
- No overnight shifts
- Hours based on facility needs
