Grace Pointe, a premier Senior Care Community located at 1919 68th Avenue in Greeley, CO, has an opening for a Community Liaison. Grace Pointe is currently accepting applications for a Community Liaison. As a Community Liaison, the primary purpose of your job position is to coordinate admission efforts for the facility assisted living areas by serving as a liaison between the facility and community referral sources. You will represent the facility in a positive way in the community, developing referral sources and soliciting inquiries and admissions to the facility. This position is Monday-Friday, 8am-4:30pm, with an on-call weekend rotation. The wage range for this position is $30.00-$35.00 per hour, and is based on years of experience as a Community Liaison.
Job Description
- Within the Social Services Department
- Review referrals from hospitals and the community, conduct the assessment process with the clinical team, coordinate acceptance with interdisciplinary team, conduct tours, complete admission pre-screen form, set up transportation, ensure insurance verification through the billing office, upload admission information to PCC, complete admission paperwork with customers, etc. to facilitate the admissions process.
- Perform as a liaison between patients, their families, and the health care continuum to ensure efficient and effective health care services are received in a timely manner.
- Clearly review with the resident and/or legal representative the financial obligations of the resident while a patient at the facility.
- Maintain relationships with case managers, physicians and the clinical teams at surrounding hospitals and clinics to ensure appropriate response time to possible concerns between the hospital and the facility.
- Maintain relationships with peer groups who are potential referral sources i.e., Assisted Living facilities, Home Care and Hospice Care as well as with other Skilled Nursing facilities.
- Make assisted living and memory care sales calls. Conduct tours and coordinate the assessment process with the Resident Care Coordinator.
- Utilize the Customer Relations Management (CRM) software in PointClickCare (PCC) to track and follow up on prospective residents and community referral sources formulating and providing continuous updates to reveal trends and indicators related to referrals, inquiries, admissions and discharges.
- Perform administrative requirements, such as completing forms, reports, etc. as requested
- Distributing facility literature and marketing materials to referral sources as requested.
