Job Title: Functional Medicine/Nutrition Health Coach
About the Role
We are seeking a compassionate, growth-minded nutritional health coach to join our functional medicine practice. This role is ideal for someone who not only understands functional medicine—but lives a healthy lifestyle and enjoys empowering patients to do the same.
Key Responsibilities
- Provide patient-centered functional nutritional coaching, including assessments, education, care coordination, and follow-up.
- Offer health coaching to support behavior change in areas such as nutrition, stress management, sleep, movement, and lifestyle habits.
- Collaborate with the provider to implement personalized care plans based on root-cause, functional medicine principles.
- Educate patients on lab results, treatment plans, supplements, and lifestyle recommendations in a clear, supportive way.
- Document visits and communication in the EHR (Cerbo experience a plus).
- Support practice workflows, including patient onboarding, follow-ups, group visits, and patient communication.
- Utilize GoHighLevel (GHL), automation, and engagement (training provided if needed).
- Model a healthy lifestyle and embody the wellness values of the practice.
Required Qualifications
- Health Coach
- Functional Medicine experience is a MUST (e.g., previous work in a functional/integrative practice, training courses, or certifications).
- Prior health coaching experience (formal health coach training, coaching certification, or demonstrable coaching experience in a clinical or wellness setting).
- Strong communication and motivational interviewing skills.
- Demonstrated personal commitment to a healthy lifestyle (nutrition, movement, stress management, self-care).
Preferred Qualifications
- Experience using Cerbo EHR.
- Experience with GoHighLevel for automation, funnels, patient communication, or CRM workflows.
- Functional medicine or integrative health certifications
- Experience in concierge, program or membership-based models.
