What are the responsibilities and job description for the Case Manager position at Grace House Inc.?
Grace House Objective: To support the clients of Grace House in gaining and maintaining housing opportunities and stability that support them in accomplishing service plan goals that resolve issues that may keep each them from accomplishing their maximum self-determination and sufficiency.
Job Objective: To support clients of Grace House through individual and group sessions with counseling and education. Assist clients with identification and resolution of all issues that would serve to prohibit self-determination and sufficiency. Work in conjunction with local community base organization for referring applicants that may qualify for program. Coolaborate with local mental health institutionals and agencies to gain referrals for those persons that meets the criteria for program to secure affordable housing. Connect with local landlords to secure affordable housing for population served and ect.
Responsibilities:
- Prepares and oversees the maintenance of all clients’ charts and case files both hard copies and clients HMIS files.
- Oversees the appropriate upkeep of progress notes, signed consents and signed contract of understanding of client handbook executed by support staff the delivery of such documentation to client’s charts.
- Ensures the appropriate documentation of client’s homelessness, disabilities, and income.
- Meets with assigned clients to develop housing/service plans as well as monthly meetings for updates.
- Seeks to educate self on housing related opportunities including availability of tax credit properties and public housing opportunities in the community.
- Assist the Directors with ensuring compliance with all funding stream requirements.
- Responsible for the needs of all assigned clients and the assistance of those clients in obtaining self-determination and self-sufficiency.
- Ensure appropriate referrals to community resources to assist client with connections to mainstream services, self-determination and self-sufficiency .
- Informs the Directors of any issues related to programming that arise or suggestions that would benefit the clients.
- Ensures the housing needs of all clients are addressed and executed appropriately.
- Upholds the confidentiality of all clients ensuring that client records and discussions of client services are kept secure and free from intrusion.
- Other duties as assigned by the Assistant Executive Director or the Director of Rental Assistance.
