At Grace Healthcare Services, we're looking for a RN Hospice Liaison to work at the RWJ Campuses to join our Interdisciplinary Team (IDT) in a contracted hospital setting. This role is a unique blend of clinical excellence, business development, and compassionate care, offering a fulfilling opportunity to support patients and families through critical transitions in care.
What You'll Do:
- Build and nurture strategic partnerships with new and existing referral sources to facilitate appropriate referrals.
- Identify and assess the psychosocial, spiritual, and physical needs of patients in the inpatient setting.
- Coordinate comprehensive care plans, ensuring seamless transitions to the appropriate level of care.
- Conduct pre-discharge hospital visits, offering support and guidance to patients and their families.

What We're Looking For:
- Registered Nurse (RN) with a valid license
- Experience in Hospice or Palliative Care is a plus
