Your Role and Responsibilities
As a Hospice LPN, you will:
Deliver Direct Patient Care: Administer nursing care to hospice patients in accordance with their individualized care plans, focusing on comfort and symptom management.
Collaborate with the Care Team: Work closely with registered nurses, physicians, and other healthcare professionals to evaluate and adjust care plans, ensuring comprehensive support for patients.
Educate and Support Families: Provide guidance to patients' families on care techniques, disease progression, and available resources, fostering a supportive environment.
Maintain Accurate Documentation: Ensure all patient interactions, treatments, and observations are meticulously recorded in compliance with hospice, state, and federal guidelines.
Participate in Continuous Improvement: Engage in team meetings and quality assessment activities to enhance the standard of care provided.
What You Need
To excel in this role, you should have:
Current LPN license to practice in NJ
At least two years of nursing experience, preferably in hospice, home care, or related fields.
Ability to effectively interact with patients, families, and the healthcare team, demonstrating empathy and clarity.
Dedication to providing compassionate care and upholding the highest ethical standards.
