Key Responsibilities
- Conduct patient and family assessments to identify needs such as food, housing, transportation, financial stability, and access to healthcare
- Identify gaps in care, including missed primary care visits, preventative care needs, and inappropriate ER use
- Document assessments and interactions in the patient record
- Coordinate outreach efforts using phone calls, text messaging, mailings, home visits, and community events
- Encourage and support patients in managing chronic conditions and engaging in preventative care
- Collaborate closely with primary care providers, care managers, and support staff to improve patient outcomes
- Address barriers to care and assist patients in accessing timely services
- Educate patients on available resources, including financial assistance, insurance programs, and sliding fee discounts
- Connect patients with community services and support programs
- Build and maintain partnerships with community organizations
Qualifications
- Completion of Community Health Worker (CHW) Training required
- Seasonal Flu Vaccine
- Experience working with diverse populations and adapting communication styles as needed
