- Manage the client intake process from initial inquiry through enrollment and onboarding
- Conduct intake screenings, assessments, and interviews with prospective clients and families
- Maintain accurate and up-to-date client records, documentation, and intake forms
- Coordinate with internal care teams to ensure smooth client transitions and service placement
- Respond promptly to phone, email, and in-person inquiries with professionalism and empathy
- Track intake data and generate reports to support organizational goals
- Ensure compliance with applicable regulations, policies, and confidentiality standards
Requirements:
- Prior experience in intake coordination, case management, social services, or a related field preferred
- Strong interpersonal and communication skills with a compassionate, client-first approach
- Proficiency in data entry, electronic health records (EHR), or case management software
- Ability to multitask, prioritize, and stay organized in a fast-paced environment
- High school diploma or GED required; associate's or bachelor's degree in human services or related field a plus
