A respected senior living provider is seeking a strong MDS Coordinator (RN) to lead assessments, care planning, and PDPM optimization-playing a critical role in both resident care and facility performance.
Responsibilities
- Complete and manage all MDS assessments
- Ensure CMS compliance and documentation accuracy
- Drive PDPM reimbursement optimization
- Develop and update resident care plans
- Conduct audits and support survey readiness
- Collaborate with nursing and interdisciplinary teams
- Active RN license (MN)
- MDS experience in LTC/SNF preferred
- Knowledge of PDPM & CMS regulations
- EMR experience (PointClickCare a plus)
