One of our top clients is seeking a dedicated and patient-centered RN Case Manager to join our team. In this role, you will play a vital part in ensuring our residents receive the highest quality of care and experience a fulfilling lifestyle.

About You :

A registered nurse (RN) with a current state license.

Minimum 1 year of experience in long-term care or a related field (preferred).

Strong assessment and care coordination skills.

Excellent communication and collaboration skills with residents, families, and healthcare professionals.

Compassionate and patient-centered approach to care.

Proficient in documentation and medical record keeping.

Ability to work independently and as part of a team.

What You Will Do :

Conduct comprehensive resident assessments to develop individualized care plans.

Coordinate care with physicians, therapists, social workers, and other healthcare providers.

Educate and empower residents and families regarding their care options.

Monitor resident health and well-being, reporting any changes to the healthcare team.

Advocate for residents' rights and ensure their quality of life.

Maintain accurate and up-to-date resident records.

Participate in interdisciplinary team meetings and collaborate to achieve resident goals.
