Job Summary : RN OR position at Gottlieb Memorial Hospital, Melrose Park, Illinois.
Experience with EPIC and PYXIS access preferred.
Schedule : 0700-1900 OR 0700-1700 Mon-Fri; call Sat / Sun prn.
Job Requirements
BLS certification required for all direct patient care personnel.
License / Certificate Verification must be dated within 30 days of start.
Fit Test required.
Trinity 9 Panel Drug Screen must be completed within 30 days of start date.
Trinity Code of Conduct Form must be signed and on file.
Trinity I9 must be completed by the agency.
Trinity Confidentiality of Patient and Proprietary Information Agreement must be signed and on file.
Trinity Specialty Specific Competency Testing must be completed within a year.
2 Step PPD OR IGRA (Quantiferon) required.
COVID-19 Vaccine proof required.
MMR vaccination proof required.
Hepatitis B vaccination proof required.
Influenza Vaccine proof required.
Varicella vaccination proof required.
Tdap vaccination proof required.
Physical / Health Statement required.

Additional Details
American Heart Association certification is required for all direct patient care personnel. Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS.
