A Shared Living Provider (SLP), is a community-based living option for individuals with an intellectual or developmental disability. A Shared Living provider supports a person who comes to live in their home with daily living skills, providing transportation to medical appointments, work, and getting out into the community. A Shared Living home becomes home for the person served.

The individual pays monthly room and board to the Shared Living provider. The subcontractor is paid through Goshen for support services covered by state funds allocated for residential services.


How can someone become a Shared Living provider?
Goshen looks for individuals that are caring, supportive and who have an extra room available in their home. It is preferred that the Shared Living provider have some experience with individuals with intellectual or developmental disabilities, but it is not a requirement. Applicants with backgrounds in human services, medical services, education, or counseling usually make great Shared Living providers.
Our goal is to create the best match possible between the subcontractor and the person desiring supports. Contractors must agree to a 12-month contract with the individual. However, it is hoped that the relationship will last much longer. Goshen ensures that the Shared Living provider meets all regulatory requirements, and provides training to provide quality services and supports.


What is Required to Become a Shared Living Provider (SLP)?

  • Must be 19 years of age or older
  • Valid driver’s license and auto insurance with reliable transportation
  • Proof of homeowner’s or renter’s insurance
  • Social Security Card
  • Proof of current pet vaccinations (if applicable)
  • Certificates for any previously completed trainings
  • Physical exam and TB test
  • Successful completion of initial and annual background checks (Criminal, APS/CPS, and Sex Offender history) for SLP, Alternate Care, and all household members over 13
  • Must be willing to support individuals with behavioral challenges, such as:
    • Property damage
    • Aggression
    • Elopement
    • Substance use
    • Other complex needs
  • Complete necessary training provided by Goshen
  • Residence walkthrough to ensure a safe, comfortable, and supportive environment
  • Strong desire to positively impact the life of an adult or youth requiring specialized services and care.



What Training is Provided by Goshen?

  • CPR and First Aid
  • Medication Aide Training
  • MANDT Behavioral Support Training
  • Orientation to Supporting Individuals with Developmental Disabilities
  • Documentation Training (Therap database)
  • Abuse, Neglect, and Exploitation Prevention Training

Major Responsibilities of a Shared Living Provider

  • Open your home to provide care and support for an individual an adult or youth requiring specialized services and care.
  • Offer transportation to appointments and activities as needed.
  • Provide a safe, nurturing, and structured living environment.
  • Promote independence by teaching and supporting daily living skills.
  • Administer medications and complete treatments as directed by healthcare providers.
  • Attend and actively participate in the individual’s team meetings
  • Ensure the person’s physical, emotional, recreational, spiritual, and social needs are met
  • Communicate regularly with the individual's team about their health and well-being
  • Complete daily documentation per Goshen and State of Nebraska requirements
  • Participate in monthly home visits conducted by Goshen staff
  • Assist with behavioral management, when applicable
