SOCIAL SERVICES REPRESENTATIVE

POSITION SUMMARY

The social worker assists with organization and execution of day-to-day social service activities initiated with Gordon Memorial Hospital District (GMHD) relative to resident care. 

CORE ACCOUNTABILITIES

1. Manages all aspects of the social services department operations effectively and efficiently.
2. Identifies and meets resident care needs in relation to the areas of arranging for social services.
3. Verifies payer source for new admissions, updating resident fact-sheets, tracking census and communicating with ombudsman.
4. Reconciles and updates families on resident trust accounts.
5. Gathers information and conducts investigations; including but not limited to resident concerns, grievances and potential abuse.  Tracks and files reports in an accurate and timely manner with the state of Nebraska.
6. Participates in the MDS, the care planning process, and PASRR screenings.
7. Performs and maintains documentation; updates policies as needed.
8. Promotes resident and staff safety in all aspects of responsibilities.
9. Processes release of information requests according to departmental policies.
10. Develops a plan that includes establishment of measurable goals, means of achieving the goals, and the progress of the individual.
11. Advocates for residents and attends resident council meetings.
12. Utilizes community resources and encourages resident participation as appropriate.
13. Participates in Social Services care education and conferences.
14. Assists in daily activities provided to residents and staff.
15. Performs other duties as assigned.

SCOPE OF IMPACT

This position acts as part of the interdisciplinary team in assessing the individual needs of the resident and participates in development and implementation of appropriate care plans.

POSITION QUALIFICATIONS

Education: 

• High School Diploma or equivalent required
• Completion of Social Service course provided by NHCA within one year of hire date required
• Associate’s degree and or Bachelor’s degree in social work or related field preferred
• Recreation therapist preferred

Experience:

• 6 months to 1 year related experience and/or training in nursing home social services and/or activities preferred

Licensure and Certification:

• Completion of 36-hour Social Service course provided by NHCA required(must complete within one year)
• Current Nurse Aide licensure in the State of Nebraska preferred
• Social Service Certification or equivalent qualifications preferred
• Basic Life Support (BLS) required
