Essential duties and responsibilities:
- Responsible for caseload assigned of consumers that are job ready and recommended by the Mission Services counselors.
- Write daily discharge summaries for referral agency/stakeholders for each consumer.
- Collaborate with supervisors, worksite staff, and participants to ensure positive outcome in Mission Services program services.
- Teach participants tasks for jobs during assessments.
- Track progress, maintain consumer contact, and provide transitional support.
- Coach and promote personal growth for each individual to reach quality standards.
- Ensure all paperwork is completed per Mission Services procedures.
Education and Experience:
- High school diploma required; Associate’s Degree preferred.
- At least one year of experience working with people who have disabilities
- Special training helpful in human relations, job development techniques, behavioral science, marketing and communications.
- Must have computer skills, including Microsoft Office.
Other Qualifications:
- Dependable, tactful, resourceful, show initiative, and has good judgment.
- Must have reliable transportation, a valid Michigan driver’s license, a safe driving record and maintain car insurance.
- Complete MDCH Direct Care Training or equivalent.
- May require lifting up to 25 pounds.
