Essential Duties:

· Actively engage as a member of an interdisciplinary team to partner with individuals to help them achieve their identified goals for life skills, job skills, and community integration. And/or assist individuals with disabilities with tasks to maintain independence in their home and community according to their individual service plan (ISP).

· Provide positive behavior support by modeling positive behavior, good manners, appropriate dress, social interactions, and appropriate language.

· Meet personal care needs of all individuals.

· Promote Goodwill within the community to enhance its public image and to obtain support for the furtherance of our mission. Demonstrate commitment to the highest standards possible in relationships with internal and external associates.

Responsibilities:

· Must be willing to provide coverage throughout our entire region. Mileage is provided if you are covering for a location that is different from where you are assigned. Our region includes Athens, Hocking, Pickaway, Ross, Vinton, Jackson, Pike, & Fayette counties.

· Assist individuals with obtaining skills and goals specified in their Individualized Service Plan (ISP).

· Ensure efforts are made to implement all individuals’ choices and preferences to promote their rights and autonomy.

· Assist individuals in all activities designed to foster independence, personal community, growth, integration, and social maturity.

· Monitor and report individual progress related to ISP outcome goals.

· Complete daily documentation for every individual who attends the center or receives services in the home. Document tasks completed, including medication administration, mileage, and hours worked in BID.

· Complete all MUI and UI documentation and notifications within required time restraints.

· Access community resources for needed support.

· Safely transport individuals in a company vehicle to and from the Goodwill Services Center and on scheduled outings. When working HPC, safely provide transportation with your personal vehicle into the community as determined by the ISP. Your personal vehicle must be insured.

· Provide personal care as needed which includes, but is not limited to, assisting individuals in and out of wheelchairs when needed and with proper use of adaptive and assistive equipment and supplies. May also be required to assist with toileting and bathing.

· Administer medication within legal guidelines and in accordance with training when needed.

· Know and adhere to the individual’s ISP, completing tasks such as light household cleaning and developing social skills in the community to cultivate better independence.

· Ensure the safety of the individual both physically and mentally. Immediately report any indications of abuse as you are a mandated reporter.

· Safely provide transportation for individuals with your personal vehicle into the community as determined by the ISP.

· Know and adhere to Ohio Department of Developmental Disabilities (DODD) rules and regulations.

· Clock in and out of Paycom for payroll at the start and end of each shift and promptly turn in mileage reimbursement to the Goodwill Services Center manager.

· May also be required to assist with toileting and bathing as determined by the ISP.

· You may be called upon at times to cover someone else’s shift to help ensure the needs of the people we serve are met.

· Complete the Ohio Department of Developmental Disabilities 8-hour provider training annually and any other required training.

· Maintain confidentiality.

· Follow all Goodwill of South-Central Ohio policies and procedures.

· Other duties as assigned.

Qualifications:

· Ability to work with all types of people, including those with disabilities. Must be able to follow instructions, have good attendance, and deal with stressful situations.

· Ability to communicate clearly and accurately by phone, email, and in person.

· Demonstrated proficiency with computers and software programs, such as Excel, Word, and cloud-based software. Proficiency or ability to quickly learn additional software, such as Paycom and BID.

· Must attain all required Ohio Department of Developmental Disabilities certifications.

· Must have open availability.

· Good organization and time-management skills.

· Pleasant personality and neat appearance.

· Will be subject to the company’s random drug testing procedures, for cause, and after accidents.

· Must pass a Goodwill-approved criminal background check and not appear on any government registries that track abusers or other such offenses.

Education and or Experience:

· High school diploma or GED

· Degree in social services and/or experience with individuals with disabilities preferred

· Prior experience with home health and/or working with individuals with disabilities preferred

Physical Requirements:

· Must be able to reach above head, bend from waist, and lift up-to 50 pounds. Accommodation will be made if the person makes use of a wheelchair or other mobility device.

Certificates, Licenses, Registrations Required:

· Current state drivers’ license, proof of current auto insurance coverage, and reliable transportation required. Ability to maintain coverage under company insurance policy for liability, bond ability and other required coverages.

· First Aid/CPR (can complete during training)

· Medication administration (can complete during training)

· Provider certification (can complete during training)
