General Purpose:
Assist Habilitation Specialist or Rehabilitation Specialist in the supervision of Rehabilitation Technicians at their assigned sites, in meeting program goals, and overseeing work quality
Essential Functions:
Education : HS Diploma/GED required, Bachelor degree from an accredited school in related field preferred.
Experience: Six months in mental health, vocational rehabilitation, or case management preferred.
Skills: Strong interpersonal & communication skills. Strong organizational and time-management skills. Ability to handle multiple tasks simultaneously and prioritize demands. Ability to work independently with minimal supervision. Flexible and open to new ideas. Approaches problems in a solution oriented manner. Avoids making assumptions about individual’s abilities and motivation. Deals with confidential information and issues using discretion and judgment .

Assist Habilitation Specialist or Rehabilitation Specialist in the supervision of Rehabilitation Technicians at their assigned sites, in meeting program goals, and overseeing work quality
Essential Functions:
- In absence of supervisor, ensure Rehabilitation Technicians are following their schedules and are at their assigned sites
- Ensure that staff are documenting services as per funding source regulations ( i.e. Medicaid and DQA)
- In conjunction with supervisor to deliver and record the implementation of services as indicated on the Individuals’ Habilitation Plans
- Assist supervisor in ensuring Rehabilitation Technicians are providing services as designed in the Hab Plans
- Monitor individual’s behavior and intervene as appropriate and inform supervisor
- Work as part of the team to ensure individuals receive seamless services provision in a variety of appropriate activities throughout the day.
- Work with supervisor or Activity Instructor to lead and coordinate daily community integration activities
- In conjunction with supervisor create schedules that are based on individuals needs, services, and staff availability
- Maintain contact with volunteer sites (site cancelations…etc.) and update Volunteer Site list regularly.
- Liaison with job coaches, individuals families, and caretakers
- Collect and record performance data from internship, volunteer, community sites
- Monitor and supervise individual sign in and out of program and ensure that information is accurately recorded in AWARDS
- Ensure compliance with DOL regulations especially around volunteering in for profits
- Report all incidents as mandated by OPWDD and NYS Regulations
- Conduct Monthly fire drills at each Hub site in the absence of supervisor
- Order supplies for each site in the absence of supervisor.
- Obtain and distribute metro cards for staff and individuals at each site. In absence of supervisor pick up and handout staff metro cards.
- Responsible for petty cash

Scope of Responsibility & Positions Supervised:
- Oversee the performance of the Rehabilitation Technicians on a daily basis.
- Make decisions on schedule changes for individuals and staff in conjunction with supervisor.
- Address performance issues with Rehabilitation Technicians and provide training. Refer issues to supervisor that require Performance Improvement plans.
- Maintain Petty Cash.
- Supervised by Habilitation Specialist or Rehabilitation specialist.
- Frequent travel & field work within the boroughs.
- Attendance at Individual's meetings that may occur in the evenings and / or at off site locations.
Salary : $21 - $22
