What are the responsibilities and job description for the Registered Nurse Assessment Coordinator position at Good Shepherd Rehabilitation?
- JOB SUMMARY
- Coordinates all state and federal requirements for completion of resident assessment, care planning, data submission, and reimbursement requirements.
- ESSENTIAL FUNCTIONS
- UTILIZES STATE & FEDERAL REGULATORY GUIDELINES AS A FRAMEWORK FOR RESIDENT ASSESSMENT , CARE PLANNING, AND FINANCIAL REIMBURSEMENT
- Established rapport with interdisciplinary team members and potential/actual residents and their families to understand the process of assessment and care planning
- Assures the timely completion of initial assessment upon admission and development of initial care plan
- Assures timely completion of on-going assessment and management of care plan on a quarterly and annual basis as well as when changing resident condition warrants.
- Assures timely submission of MDS data to the federal database
- Maintains MDS calendars for purpose of MDS completion and care plan scheduling
- Audits completed MDS’s for accuracy to support case mix reimbursement, prior to submission
- Maintains listing of resident eligibility for Medicare SNF coverage as per regulation
- Works in coordination with Director, LTC Rehabilitative Service to prepare proposed CMI reports for future fiscal years and submits same to Finance Department
- Works in concert with corporate Reimbursement Manger to prepare reports for DWP
- MAINTAINS NURSING OPERATIONS
- Assists DON in the coordination of all aspects of the LTC Nursing program, acting in her absence and/or as directed
- Maintains all aspects of HIPAA requirements
- Participates as an active member of the LTC Nursing Management Team
- Assists nurses in identifying changing resident condition and implanting appropriate follow through
- Participates in setting Best Practice standards to assure quality of care
- Supports clinical practices that adhere to the organization’s philosophy, goal, standards of care, and required governing regulations
- ASSURES QUALITY OF CARE
- By developing and interpreting LTC and nursing division philosophies and standards of care
- Enforces adherence to state board of nursing, nurse state practices, and all requirements and other governing agency regulations, such as DOH, Joint Commission, and DPW
- Demonstrates skill in understanding the range of treatment and knowledge of the principles of growth-development for the age of the residents served
- Monitors the quality.
- Indicator report on at least a quarterly basis to indicate areas of needed improvement and helps develop strategies for same with nursing management and interdisciplinary team
- MAINTAINS PROFESSIONAL AND TECHNICAL KNOWLEDGE
- Attends educational workshops, receiving and reviewing professional publications, establishing professionel networks and participating in professional societies.
- ESTABLISHES A COMPASSOINATE ENVIRONMENT
- Provides emotional, psychological, and spiritual support to residents, families, caregivers, friends, and other associates.
- PROVIDES INFORMATION TO RESIDENTS AND HEALTH CARE TEAM
- Informs interdisciplinary team of changes in regulations and in RAI manual
- Attends nurse management meetings, staff meetings, interdisciplinary team rounds, resident spirit meetings, family council (as invited), and other informal means.
- RESOLVES RESIDENT NEEDS
- Uses interdisciplinary team strategies,communicating to families, residents, caregivers as needed.
- MONITORS DOCUMENTATION OF RESIDENT CARE SERVICES
- Routinely monitors completion of forms by licensed and unlicensed staff and interdisciplinary team members to assure complete and competent results
- Assists with development of forms to adequately meet LTC regulations and the facility’s needs
- DEMONSTRATES LEADERSHIP SKILLS
- Acts as a role modeling and meeting facility objectives in personnel management
- Effectively adapts and adjusts to new or changing conditions, work pressures, or difficult situations.
- PROTECTS RESIDENTS AND STAFF
- Monitors infection control practices to determine that appropriate measures are identified implemented and documented.

- QUALIFICATIONS

- Education
- Bachelor's Degree in Nursing preferred
- Work Experience
- 3-5 years of LTC experience preferred
- 1 year RNAC experience required
- Licenses / Certifications
- Registered Nurse license required
- Basic Life Support / CPR, as a healthcare provider, per the American Heart Association required
