The Outpatient Clinical Social Worker provides psychosocial support and clinical care coordination for patients receiving psychiatric services in an outpatient setting. This role serves both adult and pediatric populations, offering assessments, counseling, resource referrals, and crisis intervention to support mental health treatment plans. The social worker collaborates with psychiatrists, therapists, medical providers, local designated agencies (such as CCS, OHP), and other healthcare professionals to ensure patients and their families receive comprehensive, patient-centered care.
The Clinical Social Worker operates under the supervision of Clinic Practice Manager & Clinic Director and plays a key role in enhancing patient well-being through advocacy, education, and psychosocial interventions.
Responsibilities
- Maintains the standards of Social Work excellence in accordance with the principles and regulations set forth by the Oregon State Health Department, DNV, and Good Shepherd policies and procedures.
- Is a core member of the clinic team which:
- Evaluates new patients , identifies problems and develops a plan of care for each patient/family through comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal directed patient care plan.
- Evaluates the course of established patients, identifies new problems and modifies the plan of care as needed.
- Assesses a patient’s financial status, as requested by the patient or family to determine eligibility for services such as Medicaid and /or assist them in getting assistance established with Senior and Peoples with Disabilities (SPD).
- Assist in referral coordination and follow up on referral completion for patients who may need stepdown or intermediate levels of care, including to regions not currently local (such as IOP, PHP, Residential programs for mental health, substance rehabilitation, eating disorders, etc).
- Performs and/or supervise patient/family psychosocial assessments of the clinic patients. Following assessment, problems are identified, and a plan of intervention is developed.
- Conducts ongoing reassessments of patient and family needs and counseling as needed or requested. Help patients and their families to maintain control and autonomy by empowering them to make their own decisions. Assist patient/families to follow the plan through counseling, information and/or referral to community resources.
- Assess factors that may complicate care giving, such as drug and/or alcohol use or violence in the home.
- Observes, evaluates, and brings to team conferences information regarding psychosocial, spiritual, physical, and financial conditions affecting the patient and family.
- Completes initial clinical contact documentation including written plan of care, clinical note within 24 hours. Documents clearly and concisely in a timely manner and secondary reports, as requested by the Psychiatrist.
- Educates community resources to the patient/family.
- Provides counseling, bereavement services or staff support within the scope of license, practice and commensurate to prior clinical experience.
- Assists in development of agency policies, program development and evaluation.
- Assists with Volunteer Training and training curriculum.
- Demonstrates continued professional growth and development through participation in education programs, required healthcare system trainings and HR updates, and review of current evidence-based health literature pertinent to social work clinical care and coordination in line with national practice standards and social work governing/regulatory bodies.
Qualifications
Education: Master’s degree in social work, from an accredited educational institution.
Experience:
- A demonstrated ability in casework, counseling and group work, including clinical work experience, preferably in a healthcare setting.
- Demonstrate experience or familiarity with the conduction of social work in prior mental health care settings
- Able to work well with others including mentally ill, cognitively/physically disabled and emotionally impaired patients of all ages from varied economic and cultural backgrounds in their home environment. This includes collaboration and interface with staff language interpreters.
- Demonstrates the ability to promote the patient’s age-specific, cultural, physical and psychosocial well-being.
- Able to work in a team setting, showing flexibility and ability to work independently when needed as well as personal.
- Able to learn and integrate new clinical and administrative skills and build an ongoing fund of knowledge (such as community resources, referral sites, etc.)

Physical Requirements:
Must be able to perceive the nature of sounds by ear. Express or exchange ideas by means of the spoken word. Perceive characteristics of objects through the eyes. Extend arms and hands in any direction. Seize, hold, grasp, turn, or otherwise work with hands. Pick, pinch, or otherwise work with fingers. Perceive such attributes of objects or materials as size, shape, temperature, or texture. Stoop, kneel, crouch, and crawl. Must be able to lift 25 pounds maximum infrequently, with occasional carrying, pushing, and pulling objects weighing less than 25 pounds. May need to be able to sit for long periods of time.
Working Conditions:
Work is primarily conducted in an outpatient clinic setting, with some mix of in-person, virtual, video, or phone-based patient interactions. May require occasional travel between clinic locations. Exposure to emotionally charged situations, including crisis intervention, high-stress patient interactions, and discussions about trauma, grief, suicide, or other complex psychosocial concerns.
