PRN Hospice Social Worker

What You Will Do As a PRN Social Worker

• Complete comprehensive psychosocial assessments and develop individualized care plans based on patient and family needs
• Provide counseling, emotional support, crisis intervention, and education to patients and caregivers
• Serve as a patient advocate while coordinating care with medical providers, community agencies, and the interdisciplinary team
• Assist patients and families with advance care planning, financial and community resources, and end-of-life decision making
• Facilitate family meetings and collaborate with chaplain, bereavement, nursing, and other team members to provide holistic care
• Maintain accurate and timely documentation in the electronic medical record
  
  

What We Are Looking For

• Master's degree in Social Work from an accredited program required (or a related Master's degree with qualifying healthcare experience)
• Two years of case management or counseling experience, including at least one year in a healthcare setting preferred
• Strong assessment, counseling, crisis intervention, case management, and communication skills
• Ability to work independently while collaborating effectively within an interdisciplinary team
• Comfort with EMR and technology
• Reliable transportation with a valid driver's license and insurance
• A heart for mission-driven, relational work with patients and families
  
  

Schedule

Schedule: PRN - we are looking for a candidate that can be available to cover about 1 shift a week.

Location: Roanoke and surrounding service areas
