We’re looking for a caring Licensed Practical Nurse (LPN) to join our Home Health team and make a meaningful difference every day.
What You’ll Do
- Provide skilled nursing care in the home setting under physician orders
- Teach patients and families about treatment plans, medications, and self-care
- Collaborate with the interdisciplinary team to ensure continuity of care
- Accurately document visits and maintain up-to-date patient records in the EMR
- Participate in on-call rotation for after-hours visits as needed
- Current LPN license in Virginia (or compact state)
- At least 1 year of nursing experience; home health or hospice preferred
- BLS/CPR certification (or ability to obtain within 30 days)
- Strong communication, critical-thinking, and documentation skills
- Reliable transportation, valid driver’s license, and proof of insurance
