Role Summary
This role involves working as part of a multidisciplinary team to provide physical therapy interventions to clients/patients/residents affected by injury or disease.
Main Responsibilities
- Develop and implement individualized treatment plans that address the unique needs of each client/patient/resident.
- Conduct examinations, evaluations, and interventions on clients/patients/residents who are affected by injury or disease.
- Utilize tools and techniques to administer physical therapy interventions following safeguards.
- Document client's/patient's/resident's information for evaluation; establish therapeutic plans and modify them if necessary.
- Educate clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home.
Requirements
- Appropriate education level required in accordance with state licensure.
- Must be licensed in the state(s) of practice as a Physical Therapist.
- When applicable, may require valid driver's license.
- Obtains and subsequently maintains required department specific competencies and certifications.
