Essential Job Duties:

1. Performs patient assessments in a systematic and organized manner to provide high quality and safe outcomes.
2. Provides direction and collaborates with team members to support a healthy work environment.
3. Analyzes collected data to formulate a plan of care that contributes to high quality and safe outcomes.
4. Implements a plan of care with measurable criteria consistent with work class performance levels.
5. Evaluates the plan of care with the patient and family/support system to maintain world class levels of patient satisfaction and outcomes.
6. Contributes to a high performance work environment by seeking educational resources that enhance and maintain current knowledge and skills.

Secondary Job Duties:

Must perform the following job duties specific to this department:

1. Includes patient and care-giver goals in plan of care.
2. Clearly identifies patient and care-giver education goals within 48 hours.
3. Attends and actively participates in Team Conferences on assigned patients when applicable.
4. Organizes work schedule to include Team Conferences when applicable.
5. Implements teaching plan and assesses need for additional patient/care-giver education at least weekly.
6. Reviews team decisions in report and with each nursing staff member involved in the patient’s care.
7. Responds to patient/family questions in a manner consistent with team decisions.
8. Documents quality measures on admission.
9. Initiates and updates treatment plans to reflect individual patient/family needs weekly.
10. All nursing staff are required to attend 70% of Rehab UBC Meetings .
    

Job Specifications:

• Education: Graduate from an accredited school of nursing; Bachelors Degree in Nursing within 3 years of employment (with the exception of staff previously granted grandfather status).
• Licensure: Registered Nurse license from the Indiana State Board of Nursing. Some areas may require a current and valid driver’s license.
• Experience: Graduate of RN Program
