- Performs patient assessments in a systematic and organized manner to provide high quality and safe outcomes.
- Provides direction and collaborates with team members to support a healthy work environment.
- Analyzes collected data to formulate a plan of care that contributes to high quality and safe outcomes.
- Implements a plan of care with measurable criteria consistent with work class performance levels.
- Evaluates the plan of care with the patient and family/support system to maintain world class levels of patient satisfaction and outcomes.
- Contributes to a high performance work environment by seeking educational resources that enhance and maintain current knowledge and skills.
Secondary Job Duties:
Must perform the following job duties specific to this department:
- Includes patient and care-giver goals in plan of care.
- Clearly identifies patient and care-giver education goals within 48 hours.
- Attends and actively participates in Team Conferences on assigned patients when applicable.
- Organizes work schedule to include Team Conferences when applicable.
- Implements teaching plan and assesses need for additional patient/care-giver education at least weekly.
- Reviews team decisions in report and with each nursing staff member involved in the patient’s care.
- Responds to patient/family questions in a manner consistent with team decisions.
- Documents quality measures on admission.
- Initiates and updates treatment plans to reflect individual patient/family needs weekly.
- All nursing staff are required to attend 70% of Rehab UBC Meetings .
Job Specifications:
- Education: Graduate from an accredited school of nursing; Bachelors Degree in Nursing within 3 years of employment (with the exception of staff previously granted grandfather status).
- Licensure: Registered Nurse license from the Indiana State Board of Nursing. Some areas may require a current and valid driver’s license.
- Experience: Graduate of RN Program
