Duties and Responsibilities
- Perform comprehensive occupational therapy evaluations in patients' homes to assess functional abilities, home safety, and equipment needs.
- Develop individualized treatment plans and establish measurable goals based on physician orders and patient needs.
- Provide skilled occupational therapy interventions to improve activities of daily living (ADLs), mobility, coordination, strength, endurance, cognitive function, and overall independence.
- Train patients in daily living skills, energy conservation, adaptive techniques, and the safe use of assistive devices, splints, prosthetics, and orthotics as appropriate.
- Evaluate the home environment and recommend modifications or adaptive equipment to promote safety and independence.
- Educate patients, families, and caregivers on treatment plans, home exercise programs, adaptive strategies, and fall prevention.
- Monitor patient progress, reassess treatment goals, and modify care plans based on clinical outcomes.
- Maintain accurate, timely, and compliant documentation, including evaluations, progress notes, and communication with the referring physician.
- Collaborate with physicians, nurses, physical therapists, speech therapists, and other members of the interdisciplinary team to coordinate patient care.
- Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policies.
- Participate in case conferences, discharge planning, and quality improvement activities.
- Stay current with evidence-based occupational therapy practices, regulatory requirements, and continuing education.
Qualifications
- Current and unrestricted California Occupational Therapist (OT) license.
- Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
- NBCOT certification preferred.
- Minimum of two (2) years of clinical experience; home health experience is preferred.
- Experience treating geriatric patients is preferred; pediatric experience is a plus.
- Strong knowledge of occupational therapy principles, anatomy, physiology, adaptive equipment, home safety, and rehabilitation techniques.
- Proficiency with electronic health records (EHR) and timely clinical documentation.
- Excellent assessment, problem-solving, communication, interpersonal, and organizational skills.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current automobile insurance in accordance with state requirements.
- Ability to travel to patient homes throughout the assigned service area.
Pay: $65.00 - $80.00 per hour
