Golden Years ADHC is seeking a compassionate and organized Life Enrichment Specialist to support participants and caregivers through in-home transitions, home visits, communication, training coordination, and ongoing service follow-up.
This is not a traditional recreation or activities position. It is a relationship-centered home-care role that combines office coordination with local travel into participants’ homes. You will help new caregivers understand the home, the participant’s approved support needs, Golden Years expectations, and how to communicate when questions or concerns arise.
You will also spend time listening to participants, helping them feel seen and heard, and making sure concerns are documented and routed to the appropriate Care Coordinator, supervisor, or case manager.
Required Qualifications
High school diploma or GED.
Strong interpersonal, communication, organization, and follow-through skills.
Ability to enter a participant’s home with respect, professionalism, and cultural awareness.
Ability to listen carefully and document facts accurately and objectively.
Comfort using email, Microsoft Office, electronic documentation systems, scanners, and standard office technology.
Ability to work independently in the field while remaining connected and accountable to the team.
Valid driver’s license, reliable transportation, and current automobile insurance.
Ability to travel locally for home visits, caregiver transitions, and service follow-up.
Ability to work full-time from the Edmonton office and in community settings.
Ability to complete all background, registry, drug, tuberculosis, training, and credentialing requirements applicable to the position.
Preferred Experience
At least one year of experience in home care, healthcare, attendant care, caregiving, care coordination, human services, social services, training, scheduling, or participant support.
Experience working with older adults, adults with disabilities, caregivers, or families.
Familiarity with Kentucky Medicaid waiver services, participant rights, person-centered care, or home and community-based support.
Experience orienting, coaching, training, or supporting employees.
CPR instructor credentials or experience coordinating CPR and first-aid compliance is helpful but not required unless specifically assigned.
What You Will Do
Help caregivers transition successfully into new participant homes using approved Golden Years handoff and orientation procedures.
Complete assigned participant home visits and make sure each person has an opportunity to share questions, concerns, preferences, and service experiences.
Explain caregiver expectations, communication procedures, schedules, participant-specific instructions, and approved service boundaries.
Confirm that caregivers understand incident reporting, emergency procedures, documentation, confidentiality, infection control, and call-off expectations.
Communicate participant and caregiver concerns to the appropriate Care Coordinator, supervisor, or conflict-free case manager.
Document home visits, transitions, attempted contacts, concerns, notifications, actions taken, and next steps in approved systems.
Coordinate CPR and first-aid training, renewal reminders, and caregiver compliance follow-up. CPR instruction will only be provided when the employee holds the required instructor credentials and is formally assigned to teach.
Help Care Coordinators maintain communication coverage while they are in the field or away from the office.
Prepare weekly and monthly calendars, visit schedules, transition plans, caregiver follow-up, and other assigned priorities.
Contact participants and caregivers before scheduled visits or transitions and confirm that the appropriate people and information will be available.
Follow up after transitions to identify concerns early and support a safe, dependable start of service.
Help maintain the assigned company vehicle, including cleanliness, organization, reporting maintenance concerns, and keeping approved supplies ready for field work.
Respond immediately to urgent health or safety concerns and follow Golden Years incident, protective-services, emergency, and escalation procedures.
Protect participant choice, dignity, privacy, independence, and freedom from pressure or retaliation.
Perform other related responsibilities within the employee’s training, authorization, credentials, and assigned role.
A Typical Day
Some days will be primarily office-based. You may begin by reviewing emails and missed calls, joining the morning team meeting, following up on caregiver credentials, preparing transition materials, answering participant and caregiver calls, and organizing upcoming home visits.
Other days will be spent in the community. You may travel to a participant’s home, complete a home visit or caregiver transition, document the visit, communicate follow-up needs, return materials to the office, and prepare the next action.
The schedule is structured, but participant needs can change quickly. The successful candidate will know how to pivot while continuing to communicate, document, and complete high-priority follow-up.

Pay, Schedule, and Growth
The hourly pay range is $20.00 to $24.00, based on directly related experience, qualifications, and demonstrated competency. Candidates with hands-on home-care, caregiver-transition, participant-support, training, compliance, or care-coordination experience may qualify for the higher portion of the range.
This is a full-time position that combines work from both our Edmonton and Lexington offices with local travel to participants’ homes and community settings.
