Job Summary:
We are seeking a highly organized and compassionate Care Coordinator to join our non-medical homemaker and companion care agency. This role is responsible for managing client admissions, coordinating caregiver schedules, and maintaining communication with case managers to ensure continuity of care and compliance with service authorizations.
Key Responsibilities:
Coordinate new client admissions, including intake, documentation, and onboarding
Schedule and manage caregiver assignments to meet client needs
Communicate with case managers to obtain and track service authorizations
Prepare and submit required 60-day reports and ongoing client updates
Maintain accurate client and caregiver records
Serve as a point of contact for clients, families, and caregivers
Ensure services are delivered according to care plans and agency standards
Qualifications:
Previous experience in home care, healthcare coordination, or scheduling preferred
Strong organizational and time management skills
Excellent communication and interpersonal abilities
Ability to multitask and work in a fast-paced environment
Proficiency with scheduling software and basic computer systems
Knowledge of non-medical home care services is a plus
Schedule:
Full-time M-F 9am - 5pm
