Golden Years Home Health, Inc is rapidly expanding and we are hiring a Home Health Nurse effective immediately. We work with patients of all ages and backgrounds who require daily care in their homes. Most of our patients are elderly so we hire the most compassionate and thoughtful talent in the field. You will be responsible for managing a heavy case load, so time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. Our Registered Nurses (RN) work in collaboration with the doctor, patient and the patient support system to provide best in class care for return to health and activity.
Essential Functions:
- Completes all assessments/evaluations, including OASIS if necessary, at specific time-points as defined by regulation and agency policy, accurately and timely.
- Develops a plan of care in collaboration with the patient, family/caregiver and physician including frequency and duration of skilled nursing visits, specific treatments, appropriate goals, other disciplines needed and discharge plan.
- Assess patients’ condition during every visit and chart your observations
- Perform evaluation tasks, including vital signs and medication review
- Administer medication as prescribed by the patient’s Physician
- Dress or redress open wounds and assess progress of healing
- Educate patients and their families on proper home health care strategies and procedures.
- Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan
- Make recommendations for devices or tools that might improve the patient’s quality of life
- Listen to the patient and respond to concerns or request.
Home Health Case Management skills:
- Nursing Assessments
- Medication Reconciliations

- IV administration and blood draw experience preferred including:
PICC lines
Portacaths
All central lines
Peripheral access
TPN

Minimum Required Qualifications:
- Minimum one (1) year experience working as a Registered Nurse
- Current/active RN License – State of California
- BLS Certification
- General knowledge and understanding of clinical verbiage
- Strong attention to detail for use in chart documentation
- Excellent time management/calendaring skills and ability to deliver documentation timely
- Strong experience using computers and computer systems
- Experience using Microsoft Suite (Word, Excel, Outlook), Gmail Suite or other systems
- Excellent written and verbal communication
Desired:
- One (1) year Home Health a plus
- Advanced computer system knowledge
- Bilingual (English/Spanish, English/Tagalog, English/Armenian/Arabic) a plus

You will be working as Independent. Rates are per visit/case basis. For our Therapists, that is $75 - $120 per case. Once you sign up to be a part of our discipline pool, you will be working hand in hand with our intake unit. The staff will be presenting you cases (depending on the demand), and it is entirely up to you to whether take or decline. Our company is very flexible, you control your own time and you may visit the patient depending on both your availability.
