CADC Group Facilitator / Case Manager
Golden State Rehab LLC is seeking a compassionate and motivated CADC Group Facilitator / Case Manager to join our growing behavioral health and addiction treatment team.
This position is currently part-time, with the opportunity to transition into a full-time role as our program continues to grow.
Responsibilities
* Facilitate psychoeducational and process groups for clients in treatment
* Maintain accurate and timely clinical documentation
* Assist with treatment planning and case management services
* Coordinate care with therapists, medical providers, families, and referral sources
* Conduct client check-ins and support discharge planning
* Monitor client progress and participation in programming
* Assist clients with accessing community resources and recovery support services
* Ensure compliance with company policies, HIPAA, and regulatory requirements
Qualifications
* Active CADC, CATC, or equivalent certification required
* Experience working in Substance Use Disorder (SUD) treatment preferred
* Experience facilitating groups preferred
* Strong communication, organization, and documentation skills
* Ability to work effectively within a multidisciplinary team
* Experience with Kipu or other EHR systems is a plus
