Responsibilities

• Provide skilled nursing care to patients in their homes
• Monitor and document patient condition and progress
• Administer medications and treatments as prescribed
• Collaborate with physicians, RNs, and care team members
• Educate patients and families on care plans and health management
• Ensure compliance with all health and safety standards
• Support infection control protocols and maintain sterile processing standards to ensure a safe environment for patients and staff.
• Educate patients and their families on health coaching topics such as nutrition, medication management, and chronic disease management.
• Participate in ongoing training related to trauma center protocols (Level I/II), ICU procedures, hospice care practices, or pediatric care as applicable.

Experience

• Active Licensed Vocational Nurse (LVN) license (California)
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication and organizational skills
• Ability to work independently and manage time effectively
• Current CPR/BLS certification (preferred or required, depending on your preference)

Pay: $38.28 - $46.10 per hour
