Role Overview
We are seeking a Licensed Practical Nurse (LPN) to join our dedicated team. This role involves providing direct, hands-on care to clients, monitoring their health conditions, maintaining detailed documentation, and collaborating closely with other healthcare professionals to ensure optimal outcomes. The ideal candidate is a compassionate, experienced nurse who values teamwork, independence, and flexibility in their workday.

Key Responsibilities
- Provide direct nursing care and monitor client health and wellbeing.
- Document patient progress accurately and maintain electronic medical records.
- Collaborate with physicians, caregivers, and community partners to coordinate care.
- Identify changes in patient conditions and communicate concerns promptly.
- Participate in ongoing training, including dementia care and fall prevention.
- Be available for on-call rotation two weekends per month.
- Scheduled working days Mondays and Fridays with the ability to cover staff vacations/time off
- Willingness to travel and transport clients
Qualifications
- Active and unrestricted LPN license (Massachusetts).
- Minimum 10 years of experience working with elderly populations.
- Background in long-term care, home care, or geriatric nursing required.
- Excellent communication and interpersonal skills.
- Strong attention to detail and organizational skills.
- Reliable transportation with valid insurance.
- Current CPR certification and up-to-date professional documentation.
- Ability to work both independently and collaboratively in a dynamic environment.
- Comfort adapting to changing patient needs and assignments.
- Experience with electronic medical records (EMR) is a plus.
