Position Summary:
We are seeking a compassionate and organized Resident Care Coordinator to join our team at the Grand Villa Assisted Living. In this role, you will coordinate and oversee the care services provided to residents, ensuring that all care plans are followed, and residents receive quality support in a safe, respectful environment. As this position does not require a license, you will work under the supervision of licensed staff and leadership to maintain compliance with Colorado Assisted Living regulations.
Key Responsibilities:
Resident Care Oversight:
Assist in the development and implementation of individualized care plans in collaboration with licensed staff.
Monitor and document resident health, well-being, and care needs on an ongoing basis.
Identify and report changes in resident condition to the licensed nurse or Director of Nursing.
Coordinate resident services such as personal care, assistance with activities of daily living (ADLs), and medication reminders.
Staff Coordination and Supervision:
Assign and oversee daily tasks and scheduling for direct care staff, ensuring proper care coverage.
Support staff training efforts by reinforcing procedures and best practices.
Ensure that care staff adhere to safety, infection control, and emergency response protocols.
Resident and Family Relations:
Serve as a primary point of contact for residents and families regarding non-clinical care matters.
Address resident and family concerns, escalating issues to appropriate leadership as needed.
Foster a positive, respectful environment that promotes resident dignity and independence.
Compliance and Documentation:
Ensure that all care services comply with Colorado Assisted Living regulations and facility policies.
Maintain accurate records, including daily care logs, incident reports, and service plans.
Assist in audits and inspections to verify regulatory compliance.
Qualifications:
High school diploma or GED equivalent.
QMAP certification.
Previous experience in assisted living.
Strong organizational and time-management skills.
Ability to communicate effectively with residents, families, and staff.
Knowledge of resident rights, privacy, and dignity requirements.
Preferred:
Certified Nursing Assistant (CNA) certification (not required).
Experience with electronic medical records (EMR) systems.
Familiarity with Colorado Assisted Living regulations.
Work Schedule:
This is a full-time position. Some evening, weekend, and holiday shifts may be required based on the needs of the residents and facility operations.
