Job Summary
We are seeking a dedicated and experienced Night Shift Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide high-quality patient care during overnight hours. The ideal candidate will have a strong background in various clinical settings, including acute and chronic care, with proficiency in medical documentation, patient assessment, and specialized nursing procedures. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using EMR/EHR systems such as Epic, Cerner, or Athenahealth.
- Administer medications and injections accurately following HIPAA regulations and established protocols.
- Manage catheterization, and sterile procedures with aseptic technique.
- Monitor patients' physiological status through telemetry and other diagnostic evaluations; respond promptly to emergent situations such as acute pain episodes or airway compromise.
- Document all patient interactions thoroughly in medical records, ensuring compliance with ICD-9/ICD-10 coding standards.
- Collaborate with interdisciplinary teams for discharge planning, case management, and post-acute care coordination.
- Provide specialized care for patients with dementia or Alzheimer's disease, including memory care and behavioral health support.
- Assist with specimen collection/processing and laboratory testing as needed.
- Educate patients and families on health coaching topics such as nutrition, medication management, and chronic disease management.
- Maintain infection control standards and ensure adherence to NCQA standards for quality patient care.
Qualifications
- Valid LPN or RN license in the state of practice.
- Experience working in diverse clinical environments such as ICU, emergency room, hospital medicine, hospice care, or long-term care facilities.
- Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks.
- Knowledge of medical terminology, physiology, anatomy, CPT coding, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.
- Skills in medication administration (including IV insertion), phlebotomy, venipuncture, suturing, and tube feeding.
- Ability to perform physical examinations and assessments accurately; experience with ventilator management or neonatal ventilators is a plus.
- Strong understanding of infection control practices and aseptic techniques.
- Excellent communication skills for working with patients of all ages—including pediatrics like infants and toddlers—and collaborating within multidisciplinary teams.
- Ability to work independently during night shifts while maintaining high standards of patient safety and quality care.
