Position Overview
Responsible for maintaining a formal comprehensive diabetes education program encompassing medical/clinical providers, physical therapists, mental health practitioners, and health educators. Monitors equipment that provides eligible patients with tools to self-manage their diabetes, reducing chronic care costs.
Key Responsibilities
Provides education and case management to patients with diabetes, prediabetes, and gestational diabetes
Maintains diabetes registries and ensures patients receive annual exams, labs, immunizations, and education per IHS Standards of Care
Coordinates primary and secondary prevention goals with the Consortium Diabetes team
Coordinates and completes the annual IHS Diabetes Audit
Utilizes audit data, reports, and RPMS tools to ensure highest quality care
Facilitates education on IHS Standards of Care to providers, dietitians, nurses, and related personnel
Supports and models the Seven Standards of Excellence
Answers inquiries from nursing staff, providers, other departments, visitors, and patients
Education, Certifications & Licensure
Valid, current RN license in the state of Alaska
Certified Diabetes Educator (CDE) or eligible to apply for certification within 2 years
Clinical Competency required within 3 months of hire and every three years
Experience Required
3 years general nursing or dietetic experience (preferred)
1 year work experience in diabetic care and management (preferred)
Required Certifications
