Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support, personal care such as brief changing.
- Monitor patient health by checking vital signs and observing for any changes in condition
- Med Reminders
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support patients with Alzheimer's, dementia, autism, or developmental disabilities through specialized memory care and behavioral strategies
- Maintain detailed documentation of patient care activities, progress notes, and incident reports in compliance with healthcare standards
Skills
- Proven experience in home health, assisted living, nursing homes, or group home environments
- Knowledge of patient observation techniques and monitoring vital signs accurately
- Strong understanding of care plans, medication administration, and HIPAA compliance
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Excellent communication skills to collaborate with healthcare teams and family members
- Familiarity with dementia care, Alzheimer's care, hospice care, and working with individuals with disabilities or developmental challenges
- CPR and First Aid certification preferred; ongoing training in patient safety is a plus
REQUIREMENTS: 1 years of professional caregiver experience, HCA Registered, Valid Current TB Test, Valid CA Drivers License, up to date car insurance and registration.
