Physical Therapist

Job Summary
We are working with an amazing Rehabilitation group to help build our team! For this role, we are seeking a creative and compassionate Physical Therapist to be positioned in assisting living facility setting as well as outpatient. The ideal candidate will provide high-quality care to patients in various settings, while thriving in a multidisciplinary team environment, communicating effectively with family members, and demonstrating personal interests in obtaining professional certifications to specialize talent. Proficiency with documentation and EMR use is a must, and candidates should be comfortable treating in the assisted living facility setting. We offer extensive training and mentorship to those without previous experience in these environments!

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations.
• Develop individualized treatment plans based on assessment findings and patient goals.
• Implement therapeutic interventions to improve mobility, strength, and overall physical function.
• Educate patients and their families about treatment plans, exercises, and home care strategies.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Document patient evaluations, treatments, and outcomes in compliance with medical standards.
• Participate in discharge planning to ensure continuity of care post-treatment.

Requirements

• Valid Physical Therapist license in the state of practice.
• Experience is preferred but not required; this is a great entry-level role.
• Knowledge of medical terminology relevant to physical therapy practices.
• Strong understanding of anatomy and physiology related to rehabilitation practices.
• Excellent communication skills for effective interaction with patients, families, and healthcare professionals.
• Ability to work collaboratively within a team-oriented environment.
• Certifications preferred but not required for: LSVT, PWRI, Skills2Care, Certified Dementia Practitioner, Geriatric Certified Specialist, Certified Hand Therapist

Pay: $90,000.00 - $100,000.00 per year
