Company Description
Diabetes Management Partners, LLC, doing business as GlyCare®, partners with hospitals to deliver specialized inpatient diabetes and glycemic management services. GlyCare translates the American Diabetes Association’s inpatient guidelines into real-world clinical practice through dedicated teams of specialty-trained Nurse Practitioners. Partner hospitals have reported measurable improvements, including reduced average length of stay, fewer insulin-related errors and serious safety events, and improved glycemic control across their inpatient population.
Role Description
We are seeking Nurse Practitioners to join our teams at 3 Catholic Health hospitals in NY. You will focus on providing clinically validated protocols tailored to each patient's unique clinical condition. Experience in diabetes management is NOT necessary. All practitioners will undergo extensive training to diagnose, treat, and manage diabetes in the hospital. New graduates are welcome.
- Practitioners will learn how to treat and manage adult hospitalized patients with diabetes and patients with poor glycemic control.
- Patients have clinical criteria for which a consult is automatically triggered.
- You are expected to see 18 to 28 patients daily, depending on the facility's active census. These visits focus only on blood sugar control and management.
- We start at 7:30 a.m. and leave when the last patient is seen. Depending on the number of providers rounding with you, the day typically ends around 4:30 ( 7—to 10-hour day).
- PRN coverage is available and will be mandatory from time to time.
- After-hours on-call responsibility is assigned on a rotating basis among all practitioners. The call can be taken from your home via phone and does not require you to return to the facility.
- Monthly RVU bonuses are paid on Productivity, Volume, and Quality.
- GlyCare covers 100% of the employee's health insurance, with the employee responsible for additional coverage. Health insurance is a PPO plan through BlueCross
- Access to additional benefits includes 401K, long-term and short-term disability, dental, and vision.
- Two weeks of paid time off (PTO)
- iPhone is required. A phone stipend of $600 is provided each year.
Shift Type
- Full-time – Three days a week and every other weekend, which equates to an average of 16 days per month (52 days less per year than a Monday-Friday position)
- Part-time - Two days a week and every other weekend, which equates to an average of 12 days per month (Full Benefits Included)

Qualifications
- Active and unrestricted Nurse Practitioner license in the state of New York.
- New NPs - Clinical hours must have been in person at a facility/office
- Medicare provider number is preferred but not required
- Previous inpatient experience in nursing or other medical fields required
- Experience as a nurse manager/supervisor or similar qualifications is a plus
- A minimum two-year contract is required
- The ability to thrive in a fast-paced environment is required
- Must be detail-oriented
- Excellent written and verbal communication skills
- Ability to be efficient while managing your schedule without immediate supervision
- Ability to quickly learn hospital electronic medical records software
Compensation
Compensation comprises a Base Salary and monthly RVU bonuses. Unlike traditional annual bonuses, our pay structure includes monthly increases based on actual census and case mix. This approach rewards individual performance and contributions, providing immediate recognition and motivation. We do not limit the dollar amount of monthly increases.
RVU Bonuses range from $3,500 to $13,000 per year. Bonuses are paid monthly with no cap.
Full-Time Position: Base salary is $118,000-$124,800 (approximately 16 shifts per month), with total compensation including benefits ranging from $140,000 to $150,000.
Part-Time Position: Base salary is $88,500- $93,600 (approximately 12 shifts per month), with total compensation including benefits ranging from $110,000 to $120,000.
Notes:
Experience in diabetes management is NOT necessary. GlyCare provides extensive training for inpatient diabetes and glycemic management.
We treat adult patients on Med-Surg and Step-down units, non-ICU level of care.
NP specializations and concentrations that are a good fit include Family Practice (FNP), Acute Care (ACNP), and Adult Gerontology (AGNP).
