Contract Details
- Position Type: Contract
- Contract Duration: 13 weeks
- Start: ASAP
- Pay Rate: $80.00–$81.00 / Hour (USD)
- Shift / Schedule: 5x8 hours; 8-hour days (facility cannot accommodate 4x10s or 3x12s)
- Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
- Travel Requirements: Traveler is expected to have their own transportation. System does a good job grouping clients to minimize drive and windshield time; occasional maximum radius of 50 miles. Mileage reimbursement at IRS Standard rate.
- Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.
- Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
Job Summary
Home Health PT position serving Medford, OR and surrounding areas. Candidate will provide skilled physical therapy services to home health patients with a standard caseload of up to 6 patients per day (30 productivity points). This position requires 1 year of skilled home health experience and is not open to first-time home health therapists.
Key Responsibilities
- Provide skilled physical therapy services to home health patients in a 1:1 ratio model
- Visit up to 6 patients per day while maintaining 30 productivity points standard
- Manage transportation to patient locations across Medford, OR and surrounding areas (occasional maximum radius of 50 miles)
- Utilize EPIC/Oasis charting systems to document patient care and progress
- Coordinate with the facility's client-grouping system to minimize drive time and windshield time
Required Experience
- Minimum 1 year of skilled home health experience required
- First-time home health therapists will not be considered
Education Requirements
- Current PT (Physical Therapist) License required
- BCLS (Basic Cardiac Life Support) certification required
- Pending license accepted
Required Skills
- Proficiency with EPIC and Oasis charting systems (Oasis experience required; please call out if traveler does not have Oasis experience)
- Ability to manage independent caseload of up to 6 patients per day
- Strong time management and organizational skills to optimize patient visit scheduling and minimize travel time
- Excellent communication and documentation skills for home health setting
- Ability to work independently in patients' homes with minimal supervision
Work Environment / Physical Requirements
- Position requires personal vehicle for transportation to patient locations
- Occasional maximum travel radius of 50 miles
- Any color scrubs acceptable
- Free parking available
- IRS Standard mileage reimbursement provided
- Home-based patient visits requiring direct patient contact
- COVID-19 vaccination (primary series plus one booster) required by facility

- Facility can call off up to one shift per pay period (every two weeks); guaranteed hours reflect this call-off policy
- Locals accepted within 50-75 mile radius at the same contract rate
- Oasis experience required; candidates without Oasis experience should confirm with hiring manager
- No weekend requirements
- No on-call requirements
- No floating between units required
