Certified Occupational Therapist (Home Health)

Job Description

• Perform patient evaluations and develop individualized treatment plans
• Provide therapy to improve ADLs, mobility, strength, coordination, and cognitive skills
• Conduct home safety assessments and recommend adaptive equipment
• Educate patients and caregivers on therapy techniques and safety practices
• Monitor patient progress and update treatment plans as needed
• Maintain accurate documentation and collaborate with interdisciplinary care teams

Requirement:

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license in the state of practice
• NBCOT certification (if required by state)
• Current CPR certification
• Minimum 1 year of Occupational Therapy experience (Home Health preferred)
• Experience with patient assessments, treatment planning, and documentation
• Strong communication and time management skills
• Valid driver's license and reliable transportation

Preferred Qualifications:

• Home Health experience
• Knowledge of Medicare and insurance documentation requirements
