MDS Coordinator
Glenwood Springs, CO | Full-Time | $42–$50/hour
Glenwood Springs Health Care is looking for an experienced MDS Coordinator to join our clinical team.
Our facility supports residents with a wide range of complex nursing and rehabilitation needs, along with specialized clinical programs and onsite therapy. As our MDS Coordinator, you’ll have ownership of the MDS process while working directly with nursing, therapy, and facility leadership.
This is a highly involved role within the facility. Your work has a direct impact on care planning, quality measures, reimbursement, and ensuring the care our teams provide is accurately captured. You’ll have the opportunity to see the full picture of what is happening clinically and help bring nursing, therapy, documentation, and care planning together.
