Location: Glenview Professional Pharmacy, Richland Hills, TX 76180

Position: Full-Time Pharmacist Schedule: Monday–Friday with rotating weekends/on-call.

We are seeking a compassionate, detail-oriented, and motivated Long-Term Care Pharmacist to join our growing pharmacy team. Position Summary

The Long-Term Care Pharmacist is responsible for reviewing and dispensing medications, providing clinical support to healthcare providers and facility staff, and ensuring the safe and effective use of medications for residents in skilled nursing and long-term care facilities.

As part of our pharmacy team, you will work closely with fellow pharmacists, pharmacy technicians, nurses, and providers to deliver exceptional patient care while maintaining compliance with all applicable regulations and standards.

Responsibilities

  • Review and verify medication orders for accuracy, appropriateness, and regulatory compliance.
  • Evaluate medication therapies for potential interactions, allergies, contraindications, and dosing concerns.
  • Communicate with physicians, nurses, and facility staff regarding medication orders and clinical recommendations.
  • Provide drug information and clinical support to healthcare professionals and caregivers.
  • Assist in identifying and resolving medication-related problems.
  • Support medication safety and quality assurance initiatives.
  • Ensure compliance with federal and state pharmacy regulations and long-term care standards.
  • Maintain accurate documentation and patient records.
  • Participate in pharmacy operations and workflow management.
  • Participate in pharmacist on-call rotation as assigned.

Qualifications

Required:

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree from an accredited school of pharmacy.
  • Active and unrestricted pharmacist license in the state of practice.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work effectively in a fast-paced, team-oriented environment.
  • IV certification.
  • Immunization certification.

Preferred

  • Previous experience in long-term care, institutional, or hospital pharmacy practice.
  • Familiarity with long-term care regulations and pharmacy operations.
  • Experience working with pharmacy software and electronic medical records.

Salary : $120,000 - $140,000
