ESSENTIAL JOB DUTIES

1. In an individual, group or telehealth setting, provides nutrition assessment, diagnosis(es), intervention and monitoring to primary care patients referred by providers of the health centers. Determines or recommends nutrition prescriptions and initiates interventions based upon the diagnosis(es) established.
2. Provides patient-centered care and utilizes client centered communication principles in line with the mission of the organization.
3. Documents all work performed and outcomes of nutrition interventions in electronic medical records (EMR) in a timely manner as required by GLBHC Policy.
4. Collaborates and communicates with members of the integrated care team to advance nutrition related care. Participates in case conferences, huddles, meetings or other activities to contribute to coordination and quality of patient services.
5.Assesses the need for patient education materials and other forms and assists in the development or procurement of these materials.
6. Assesses patient’s need for other health services and makes appropriate referrals.
7. Acts as a resource to all health center staff by staying current with nutrition knowledge, standards and practices of care.
8. Assists with the development, implementation, and review of nutrition informatics and department policies and procedures.
9. Participates in Quality and Process Improvement (QI) efforts, as needed.
10. Participates in community events and collaborations in efforts to broaden awareness of GLBHC mission and its services, and with goal of improving the overall health of the community

MARGINAL JOB DUTIES
- If bilingual (English/Spanish) translates education materials and interprets for providers when necessary.
- Assist WIC Program if trained in the MI-WIC system.
- Performs other duties as assigned by the Primary Care Nutrition Services Coordinator.
JOB SPECIFICATIONS
- Qualifications: Currently a Registered Dietitian (R.D.) or Registered Dietitian/Nutritionist (R.D.N.) with the Commission on Dietetics Registration/Academy of Nutrition and Dietetics with a Bachelor’s degree or advanced degree from an accredited institution with a major in Foods and Nutrition or Dietetics. BLS Certified.
- Experience: One year of related job experience preferably in public or community health/patient education
- Interpersonal Skills: Ability to communicate effectively with a diverse population. Punctual, organized, dependable and flexible in nature. Ability to work as part of a team.
- Physical Effort: Medium
- Hours: Full-time or Part-time, as scheduled. Flexible and varied. Late afternoon and evening hours may occasionally be required.
- Travel: If travel to sites not normally assigned is required reimbursement consistent with GLBHC policy if personal vehicle is used.
