Position Summary: The Psychiatric Rehabilitation Services Worker provides group and/or individual skill development interventions with adults 18 years and older who are diagnosed with a serious, persistent mental health disorder.  Services are focused within the Living, Social, Vocational, Educational, and Wellness Domain to empower individual’s ability to manage the severity of their mental health issues while living, learning, working, and being social within their natural communities.  Services are provided at the site-based location, community and individuals’ homes.   This position is primarily mobile (in consumer's home and/or community) but at times, site-based services within the Butler office may be required. Since services are provided individually in the consumer's community and/or home environment, travel is required to these locations.
 
QUALIFICATIONS:
  1. A Psychiatric Rehabilitation Services (PRS) Worker shall have one of the following: 
    • Bachelor’s degree with current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification preferred;  OR 
    • Bachelor's degree; OR
    • An associate's degree and one (1) year work experience in mental health direct service; OR
    • A Certified Peer Specialist (CPS) certificate and one (1) additional year paid or volunteer work experience in mental health direct service; OR
    • A high school diploma or GED and two (2) years’ work experience in human services which must include one (1) year of mental health direct service. 
  2. Must have acceptable PA State Police Clearance, PA Child Abuse History clearance, and FBI clearance.
  3. Valid driver’s license and acceptable driving record is required.  
ESSENTIAL DUTIES OF THIS POSITION
Service Delivery
  1. Provide direct services in alignment with Glade Run’s mission: Founded in Faith and Grounded in Excellence, We Empower Individuals to Experience Personal Wellness and Growth.
  2. Model trauma-informed, culturally responsive, and strength-based practices in all direct work with individuals and families and collaboration with team members.
  3. Focus on empowering individuals to achieve greater independence through skill building, development of positive daily routines, strengthening coping skills, and supporting individualized goals that reduce reliance on professional systems and services.
  4. Hold individuals accountable to program handbook requirements, expectations, and established program rules while supporting engagement and progress toward individualized goals.
  5. Make day to day decisions regarding cases in the program and keep the Program Leadership informed of high-risk cases, pertinent issues related to clients and contracting agencies.
  6. Build relationships and communicate with County Human Services providers and other community referral sources. Making referrals to community supports as appropriate. 
  7.  Engage in goal oriented, purposeful, intentional interactions with clients.
    • Implement the daily schedule of the Psychiatric Rehabilitation Services program for site based and/or mobile programming.
    • Carry a full caseload and provide direct care in accordance with the following requirements; Individual Recovery Plans in collaboration with individuals and their preferred representatives. Complete   Plans in a timely fashion in accordance with regulations and agency policies.  Involve family members and stakeholders in the service planning process and indicate participation through obtaining signatures of all participants.
    • Provide group and/or individual curriculum that will help individuals learn to become active in the community, gaining independence in use of public transportation, budgeting and shopping, and accessing social, spiritual and recreational outlets.  
    • Complete a psychosocial history and all required documentation such as admissions, policies and consents, handbook review, release of information, crisis plans, for all clients within 30 days of admission to Glade Run Lutheran Services. 
    • Complete initial and ongoing assessments using tools like SBA (Strengths Bases Assessment.  Collect ongoing data and help analyze patterns, trends, and monitor progress/regression
    • Obtain and complete the service history information, initial assessment material, encounter forms, service documents, and the individualized rehabilitation plan in a timely and accurate manner.
    • Assist consumers with accessing appropriate community services, living in the least restrictive setting, obtaining basic necessities, and conducting activities of daily living.
    • Write progress notes that are completed in a timely manner with all required components. 
    • Provide discharge planning and follow-up, beginning with planning at the time of admission. 
    • Instruct/coach and support individuals utilizing interventions and skills that support the individual client’s recovery plan.
Risk and Safety Management 
  1. Support timely reporting, tracking, and documentation of critical incidents and unusual occurrences in accordance with regulatory and agency policies.
  2. Assist program leadership with follow-up actions related to incidents, including gathering documentation and participating in review processes as requested.
  3. Maintain working knowledge of child and adult protective service laws and reporting requirements, including ChildLine, county protocols, and internal documentation procedures.
  4. Provide Supervisor with up-to-date information regarding on-going programmatic, quality, authorization and compliance issues
Supervision and Service Documentation Coordination
  1. Meet twice a month for supervision
  2. Complete monthly self and peer chart reviews to be discussed in supervisions
  3. Attend and Participate in Monthly Team Meetings
  4. Attend and Participate in Quarterly Program Meetings
  5. Complete all progress notes within 72 hours of the session.  All documentation must be completed for billing by staff by the end of work week.
  6. Manage site-based and mobile caseload, averaging 15/25 individuals per staff,  ensuring all assessments, rehabilitation plans, ROI’s, Policy and Consents, and any additional documentation is up to date and  completed in accordance to agency policy /program expectations and compliance.  This includes supporting other PRS team members with their caseload as needed.
  7. Work collaboratively with the PRS team to facilitate weekly site based and mobile sessions

$19.25-$20.25 hourly based on education and experience
