Part-Time Day Shift Social Worker
Days (32 hours per pay period)

You'll work alongside a caring interdisciplinary team to advocate for resident needs, coordinate care, and ensure each individual receives the resources and guidance needed to thrive.
What You'll Do
- Assist with resident assessments, care planning, and discharge planning.
- Conduct cognitive and mood evaluations and coordinate follow-up services as needed.
- Provide emotional support, guidance, and education to residents and families.
- Connect residents with community resources, financial assistance programs, and social service agencies.
- Maintain timely documentation and collaborate with the care team to support resident well-being.
- Licensed Social Worker (LSW) in the State of Michigan.
- Bachelor's or Master's degree in Social Work or a related human services field.
- Healthcare social work experience preferred.
- Experience working with older adults or in long-term care preferred.
- A compassionate, resident-centered approach and strong communication skills.
