Job Summary
Gilroy Healthcare Center is seeking a detail-oriented and organized MDS Assistant to support the MDS department in coordinating resident assessments, maintaining accurate documentation, and ensuring compliance with state and federal regulations. The ideal candidate will work closely with nursing staff, therapy teams, and administration to help support quality resident care and reimbursement processes.
Responsibilities
- Assist with completion and coordination of Minimum Data Set (MDS) assessments
- Gather and organize resident documentation for care planning and reimbursement purposes
- Support the MDS Coordinator with scheduling and tracking assessments
- Communicate with nursing, therapy, dietary, and social services departments regarding resident care information
- Maintain accurate and timely records in compliance with CMS guidelines and facility policies
- Review charts and documentation for completeness and accuracy
- Assist with care plan preparation and interdisciplinary meetings
- Perform administrative duties related to the MDS process as assigned
Qualifications
- Experience in skilled nursing facility (SNF) or healthcare setting preferred
- Knowledge of MDS process and Medicare/Medicaid documentation preferred
- Certified Nursing Assistant (CNA) or healthcare administrative experience is a plus
- Strong organizational and communication skills
- Ability to maintain confidentiality and handle sensitive information professionally
- Proficient with computer systems and electronic medical records (EMR)
- Ability to work collaboratively in a fast-paced healthcare environment

Pay: $50.00 - $55.00 / hour
