Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record.
- Administer medications and injections accurately following established protocols, including IV infusions and medication management for acute and chronic conditions.
- Provide wound care, ostomy care, and catheterization with strict adherence to aseptic techniques and infection control standards.
- Support patients with complex needs such as dementia, memory care, behavioral health issues, or those requiring hospice or palliative care.
- Document all patient interactions thoroughly in medical records while maintaining HIPAA compliance.
- Support laboratory specimen collection, processing, and basic diagnostic evaluations to aid in timely clinical decision-making.
- Educate patients and families on health coaching topics such as medication administration, nutrition, pain management, and discharge planning.
