We are currently hiring for a Registered Nurse (RN) position! This is a .9 FTE with day shifts, for our Phalen, St. Paul outpatient Clinic. The outpatient RN will support Physical Medicine and Rehabilitation providers in an adult specialty care clinic, support clinic visits and procedural-based care including (e.g. Botox/Dysport/Phenol Injections, Baclofen pump management and refills, sedation monitoring and procedure support). Experience in sedation monitoring and procedure support highly preferred. No nights/weekends or holidays. Benefit eligible!


The RN will support clinic visits and procedural-based care including (e.g. Botox/Dysport/Phenol Injections, Baclofen pump management and refills, sedation monitoring and procedure support).

  • Provide essential clinical triage and rehabilitation-focused patient education
  • Coordinate multidisciplinary care and community resources
  • Patient triage for pain, neurologic, and functional concerns
  • Care coordination with therapy services, interventional providers, and external agencies
  • Medication management (e.g., spasticity agents, pain medications)
  • Pre‑ and post‑procedure education and follow-up (e.g. Botox/Dysport/Phenol Injections, Baclofen pump management and refills, sedation monitoring and procedure support)
  • Durable medical equipment (DME) coordination and prior authorizations

    This position exists to administer family-centered nursing care to patients, advise patients on health maintenance and disease prevention, and/or provide skilled assistance in navigation of the health care system as appropriate to the developmental, cultural, emotional, and medical, surgical, or rehabilitative needs of the patient and their families.

    Specific duties are determined by the work setting or patient population served; however, in all positions, serving patients with challenging and complex conditions requires a concentration of specialized staff able to work in highly coordinated teams. We organize care around each patient’s condition which requires staff at multiple levels and across many departments to exhibit exceptional teamwork qualities including communicating efficiently and effectively and adopting the behaviors required for high functioning integrated teams.

     

    Compensation & Benefits Information

    The hourly wage for this opportunity is $39.86 to $68.13. Pay is dependent on several factors including number of hours worked within the field, education, and certification/licensure. Hourly pay just one part of the compensation package for employees.     Core Responsibilities and Duties  

    Patient Care – Delivers patient care which optimizes the patient’s health status and

    outcomes using the nursing process.

    • Demonstrates thorough assessments.
    • Creates individualized plans of care that are evidence-based.
    • Successfully implements interventions.
    • Completes timely, accurate and thorough documentation.
    • Effectively prioritizes.
    • Evaluates patient care based on best practices.
    • Communicates patient-related nursing data and information to enhance active and informed decision making by patients, families, nurse colleagues, and other healthcare providers.
    • Manage between visit patient needs. 
    • Care coordination 
    • Prepares for clinics through chart prep, which includes review of current and outside records, images, and communication regarding the plan of care for patients.

    Family centered care - Provides nursing care and health education reflecting specific patient, family, cultural and community needs.

    • Identifies the learning needs of the population served.
    • Implements an effective teaching plan.
    • Provide specialty focused patient education
    • Employs strategies to promote health in a safe environment.
    • Seeks opportunity to evaluate effectiveness of health education as culturally and developmentally appropriate. 
    • Adapts to staffing and clinic workflow needs based on patient care needs and availability of staff, space, and services.

    Autonomous Practice – Exercises independent judgment that influences care and demonstrates understanding of potential clinical implications.

    • Supervises, delegates, and provides care based on the abilities and limitations of care personnel and patient acuity.
    • Identifies and incorporates evidence-based improvements into own daily work.

    Procedures and Policies –Identifies, interprets, and implements organizational policies and procedures according to regulations, professional and community

    standards.

    • Protects the confidentiality of personal health information.
    • Holds self and others accountable to a healthy work environment.
    • Maintain professional boundaries.
    • Must follow infection prevention practices.

    Active Engagement – Actively represents the department in committees, task forces, unit activities or other work groups and disseminates information and

    decisions to colleagues and peers using the Engaged Professional Practice Model.

    • Attend a minimum of 75% of department staff meetings.
    • Attend a minimum of 75% of committee meetings.
    • Demonstrates meaningful contributions to the productivity of the professional environment of the department.
    • Supports work of Clinical Shared Governance initiatives.

      Professional Development and Quality Improvement - Participates in activities that promote professional growth, ease of access, quality improvement and patient satisfaction.

      • Maintains professional competence by pursuing opportunities for professional growth.

      • Performs chart audits and review as needed for Quality Improvement compliance and metrics.

       

      Qualifications  

      Required

      • Documentation of completion of professional nursing program (RN program)
      • Current Registered Nurse (RN) license through the MN Board of Nursing
      • Current American Heart Association Basic Life Support (BLS) certification. If candidate holds valid CPR through another acceptable organization (as determined by Gillette), candidate may be able to begin employment but must complete Basic Life Support through the American Heart Association within one month of employment.
      • Must be interested in working with people with short-term or long-term disabilities that begin in childhood

      Preferred

      • Bachelor’s degree in nursing or other healthcare related field
      • Prior nursing experience in a health-related setting
      • Nursing Specialty Certification relevant to the position (ex. Pediatrics, Palliative & Holistic Nursing, Care Management)
