We are seeking a Nurse Intake Coordinator to join our growing home health team.
In this crucial role, you will manage the initial patient intake process, ensure seamless transitions into our healthcare services, and serve as the primary communication bridge between patients, families, and healthcare professionals.

Qualifications:
- Documentation: Current Texas Driver’s License.
- Registered Nurse Certification
- Experience: 1 years of scheduling experience in Home Health (Preferred).
- Work History: Strong professional background with excellent references.
Skills:
- Sharp attention to detail with a strict commitment to patient confidentiality (HIPAA).
- Strong understanding of inpatient and outpatient care processes.
- Exceptional communication and interpersonal skills.
- Familiarity with ICD-9 coding standards is a plus.
Job Duties & Responsibilities
- Manage Referrals: Answer incoming referral calls promptly and professionally.
- Data Entry: Accurately record and enter basic referral information into our Excel system.
- Insurance Verification: Verify patient insurance information and secure initial authorization for services.
- Coordinate Care: Communicate referral details to the Clinical Coordinator/Assistant and track case assignment responses.
- Process Admissions: Input customer data, admission information, and referral notes into the electronic database upon case acceptance.
- Manage Declinations: Professional contact referral sources if a case is declined, explain the reasoning, express gratitude, and log the details in the system.
- Track Capacity: Maintain an updated status of agency availability and proactively communicate this to referral sources.
- Administrative Support: Assist with census reporting and identify potential new referral streams.
- Other Duties: Perform additional operational tasks as assigned to support the team.
