Minimum Requirements
- Must be a licensed LPN in CT
- High school diploma or equivalent, Associate’s or Bachelor’s preferred
- Experience working with individuals with substance use disorders and psychiatric issues
- Five years of clinical experience
- Must maintain and provide proof of valid driver’s license and automobile insurance in good standing.
- Reliable use of personal vehicle as needed

Responsibilities
- Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders as well as medical knowledge related to health and well-being
- Supporting the Program Director in leading an effective program through clinical services and best practice models of care
- Maintaining contact with MDs (i.e. psychiatrists, PCPs, specialists, etc.) to coordinate medical and psychiatric care of individuals
- Working closely with pharmacies to facilitate proper pharmaceutical care
- Providing guidance and peer-support regarding medical care in their respective program
- Conducting Health Histories, Health Assessments, and medical evaluations of clients’ emergent medical issues
- Making appropriate community referrals based on assessments and evaluations to meet clients’ basic health needs
- Providing ongoing medical education to case managers informally and in training formats
- Ensuring proper orientation, training, and management of the medication self-administration system
- Completing all required documentation electronically and on paper in a timely manner
- Acting with compassion, integrity, and ethics
