What You'll Do:
- Assist with resident assessments, care planning, and required documentation
- Support residents and families during admissions, transfers, and discharge planning
- Help residents adjust to long-term care and promote a positive living environment
- Advocate for residents' rights, dignity, and overall well-being
- Serve as a liaison between residents, families, and the interdisciplinary care team
- Assist with coordinating resident-centered programs and activities that promote emotional and social wellness
- Maintain accurate, timely, and confidential documentation
- Help connect residents and families with appropriate community resources as needed
- Support compliance with state, federal, and company regulations
- High school diploma or equivalent required
- Associate's or Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred
- Previous experience in skilled nursing, long-term care, healthcare, or social services preferred
- Excellent communication, organizational, and interpersonal skills
- Strong problem-solving abilities and attention to detail
- Compassionate, empathetic, and resident-focused mindset
- Ability to maintain confidentiality and professionalism in all interactions
