What are the responsibilities and job description for the Community Health Worker position at Gifford Health Care?
Gifford is seeking a Community Health Worker to join our Blueprint department.
The Community Health Worker (CHW) is responsible for increasing access to health care, mental health support, community support, and social services for clients who receive services from Gifford Health Care. The CHW will promote health by assessing the client’s needs and providing information, support, and education to facilitate clients to make their own decisions to practice self-management. The CHW functions as the facilitator of the Core Team and works with providers, patients, clinical staff, office staff and expanded Community Health Team resources to identify patients with or at risk for chronic conditions, identify social determinate of health need, and/or mental health care and support. The CHW will work closely with patient and provider to establish a plan of care to improve the patient’s physical and mental health, quality of life, and self-management. The CHW will work with the other Core Team members to address barriers to self-management, referring to an expanded Community Health Team as needed, i.e. dietitian, mental health services, home health services, Community Action, Agency on Aging, etc. This position requires critical thinking skills to plan and coordinate care. The CHW takes an active role in the management of patients with or at risk of chronic conditions, identified social determinate of health need, and/or mental health care and support as well as providing support and guidance to providers, clinical staff, office staff and others.
Requirements:
- Minimum of GED or High school Diploma required
- Some college or degree in a related field preferred
- 2–4 years in care coordination, case management, or other social service support role
