About the Role:
In anticipation of future government awards, GiaCare Inc. is now accepting applications for Registered Nurse Case Managers to support Naval Health Clinic (NHCQ) in Quantico, Virginia.
Education and Experience:
- Bachelors of Degree in Nursing; Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
- One year of experience in nursing
Certifications:
- Basic Life Support (BLS)
- Possess one of the following certifications: Commission for Case Manager Certification Certified Case Manager (CCM), Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS), Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN), American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S), National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC), Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC), American Nurses Credentialing Center Nurse Case Manager (RN-NCM), National Academy of Certified Care Managers: Care Manager Certified (CMC).
Duties:
Provide full-spectrum case management services in coordination with multidisciplinary healthcare teams
Assess, plan, implement, monitor, and evaluate individualized care plans to optimize health outcomes
Advocate for patients by guiding them through treatment decisions and the healthcare system
Track and report the medical status of assigned personnel, especially those on light or limited duty, TLD, PLD, or in DES/IDES processing
Maintain and update detailed case files and ensure compliance with HIPAA and administrative requirements
Monitor re-evaluation timelines and coordinate with MTFs, Patient Admin, GPAC, HQMC, and CMC (MMSR-4)
Participate in MCESG Force Preservation Councils and provide command-level briefings on readiness status
Ensure accurate and timely documentation across CHCS, AHLTA, MHS Genesis, MEDBOLTT, SMART, MOL, MCTFS, MMRS, DRRS, and related systems
Facilitate specialty referrals, arrange care coordination, and support transitions through IDES
Train and advise clinical staff and leadership on case management processes and patient readiness
Maintain credentials and continuing education, and comply with infection control, safety, and performance standards
Support implementation of performance improvement and family advocacy programs as needed
Respond effectively in emergency situations and ensure ethical, risk-aware patient care delivery
